An international survey for end users of NeurOptimal® Dynamical Neurofeedback®

The Survey
You will be asked to answer questions about basic demographic information, your goals and hopes, your well-being and daily functioning, your experience with sessions and training as well as your satisfaction.

The survey is available in English, French, Dutch, Italian, and Spanish and consists of a two-part questionnaire. The first one will be filled out at the start of your series and the second one after completing your Training. They will respectively take about 10 and 20 minutes to fill out. Set aside a time when you can answer your questions thoughtfully.

Because we know how precious time is, we want to thank you by giving you a graphic representation of some of your responses at the end of the second questionnaire allowing you to view your own data. 

How does it work  
Your NeurOptimal provider will give you the access codes and guidelines for the questionnaires. 

Anyone can participate. It also does not matter if this is your first series of Sessions or if you have done Training before. 

All we ask is that you fill out the 1st questionnaire when you start your series of Sessions. Since we'd like you to assess 'your baseline' at the moment you want to start Training

You may only submit one survey for yourself however if you are a guardian or parent, you may also submit a survey for a child or person under your care. 

Data Collection
We have contracted services with a third-party Dutch bureau, Triqs, to conduct this survey and analyze the results. The data collected will be stored in a secure location. Your data will be confidential and will not be sold or used for anything other than this survey.
